Community Information Exchange (CIE) San Diego was recognized as a Live Well San Diego partner on December 11, 2014. CIE San Diego launched an innovative trust network and collaboration catalyst that provides the connective tissue to enhance care coordination among social service and care providers by enabling them to easily share client-level information. The initial pilot addressed the needs of homeless service provides enabling more than 75 users across six organizations to access program history and case manager information for more than 800 homeless clients in downtown San Diego. With a second grant of $1 million from the Alliance Healthcare Foundation, CIE will expand to organizations serving seniors aging in the community. CIE San Diego is a key component of the county’s regional information exchange infrastructure. Over time, CIE San Diego aims to serve thousands of San Diego County social service nonprofits and continue to build collaborations with San Diego Health Connect and the County of San Diego's Knowledge Integration Program.
