360-Degree View Will Enhance Client Experience and Outcomes

Every hour of every day, people in San Diego County are searching for services and resources to build, improve and sustain healthy lifestyles. With thousands of services, programs and resources in our community, 2-1-1 San Diego offers deeper coordination services that go beyond simple information and referral. With more than 10 years serving 1.5 million clients, 2-1-1 San Diego has a pulse on evolving service systems to ensure that each client is receiving the best care possible and alleviating barriers to building healthy lives. To take their service delivery to the next level, 2-1-1 San Diego has launched the 360o Community Coordination platform.

In collaboration with their service provider partners, 2-1-1 San Diego is developing an interactive, shared client record to provide a longitudinal view of a client’s history, needs and services received. This 360-degree view gives partners an in-depth understanding of a clients’ unique experience, enhances the client experience and improves shared outcomes. 

Through the platform, partners will have access to shared client information, providing a complete view of how each service provider is helping each client.  This will foster formal relationships with local service providers, leveraging each other’s’ expertise, reducing duplications and providing shared language and measures of success.  In addition, this system will allow for proactive and agile care coordination including bi-directional referrals. Each partner will receive real-time feedback reports from other participating partners serving the same client.  The 360o Community Coordination platform will allow partners to work collaboratively within one ecosystem to provide high-quality, actionable data through shared individual client records with outcomes that indicate unmet needs, as well as each organization’s impact on each client’s health and social functioning over time.

Building a Stronger Community Means Building Awareness

This year 2-1-1 San Diego is doubling its awareness in San Diego County so everyone will know about the 2-1-1 service. With February historically being “National 2-1-1 Awareness Month”, 2-1-1 San Diego is launching a “28 Days of Impact” campaign that will kick off their year-long campaign to work with partners, supporters and volunteers to promote and raise awareness of 2-1-1 throughout the San Diego region.
