Health Center Partners

Health Center Partners of Southern California (formerly Council of Community Clinics) joined Live Well San Diego as a recognized partner on May 5, 2015.  As a recognized Live Well San Diego partner, Health Center Partners will continue to create healthier San Diego communities, especially among the vulnerable, low income and uninsured.

For the past 38 years as the former Council of Community Clinics, Health Center Partners has represented and supported community clinics and health centers in their efforts to provide access to quality health care for the diverse communities they serve, with an emphasis on low income and uninsured populations.

Health Center Partners' members represent hundreds of practice sites throughout San Diego, Imperial and Riverside counties. They serve over 750,000 patients annually and provide two million unduplicated patient encounters.

Health Center Partners is invested in an ongoing partnership and commitment to Live Well San Diego by promoting healthy lifestyles in the diverse communities served by its member health centers and community clinics.

Health Center Partners has numerous initiatives that support Live Well San Diego, including:

  • The Integration Institute, which promotes tobacco and drug/alcohol free communities by linking community and county behavioral health providers with its member’s primary care providers
  • A rigorous outreach and education campaign to improve access to health care coverage
  • Collaborations with hospitals and other partners to reduce readmission rates

Health Center Partners is committed to continuing to advance Live Well San Diego into the future, and is embracing strategies of this initiative in ways illustrated here:

  • Building a Better Service Delivery System by implementing cross-system behavioral health integration strategies, like care team conferencing for shared treatment plans and transitioning clients across the system
  • Supporting Positive Choices by working with community health centers to promote healthy eating and better nutrition among patients
  • Pursuing Policy and Environmental Change by working with congressional and legislative delegations to ensure the needs of health centers are represented in Sacramento and Washington, DC
  • In other ways by creating a healthy and resilient San Diego community, especially among vulnerable populations
