UPLIFT- Urban People Living in Faith & Trust joined Live Well
San Diego as a recognized partner on November 28, 2017. UPLIFT
has collaborated with business and community leaders for 30 years to
improve the quality of life for the vulnerable, low-income and
racially diverse populations of San Diego. Their programs deliver an
average 30,000 volunteer hours per year, providing support in two
primary areas student and homeless services. Their main programs
• Kids at Heart –8 weekly, literacy focused, academic tutoring for children in grades K - 12
• Swimming Upstream –a 15 week program that develops leadership skills and encourages positive life choices for young women grades 7-12.
• The Home Team – assists Professional Service Providers assess Individuals experiencing homelessness, rank for vulnerability to death on the streets, assist with Benefit Coordination and Housing Navigation, and Companionship to help new residents "settle-in" to their new homes. Those served include Shelter Residents and street Homeless including Vets, Disabled Adults, Survivors of Domestic Violence & Displaced Families with Children, Transitional & Interim Program participants and Permanently housed residents in Permanent Supportive or Long-term Subsidized Housing.
• Triple Cross –Provides California ID's, necessary for re-entering the working world and receiving important social services, to Neil Good Day Center Clients Tuesday & Thursday mornings. UPLIFT assists with the Homeless Verification and application for Free CA IDs and provides transportation to the DMV.
