COMMUNICATING COMMUNITY PLANNING EFFORTS AND PROGRESS IN A POWERFUL WAY

 3x1 Community Enrichment Plan and Scorecard story - North County Community Leadership Meeting

Our regional Community Leadership Teams use a data-driven, outcomes-focused, and community-centered process to prioritize health and safety issues in our communities. The process requires collaboration and partnership and is based on the principle that everyone’s concerns and ideas should be heard, and the community should drive and take ownership of the resulting plan.

Data-Driven

First, a Community Health Assessment is completed by collecting information and data to help educate the Teams about the status of each region’s health and safety metrics, including presentations on health statistics, demographics, and social drivers of health such as income, education, and access.

Outcomes-Focused

A Community Enrichment Plan is then developed based on the community health and safety needs identified by the Assessment. The Plan helps government staff and community members work together to set priorities, direct the use of resources, and outline actions to address strategic focus areas that can improve community health and safety over the long term.

Community-Centered

Together, our Recognized Partners work with local leaders and community members, alongside County of San Diego staff to form workgroups and plan specific actions in support of each priority area. Partners from all sectors and backgrounds are needed to help create innovative solutions to address our communities’ most pressing needs.

 

 

Regional Scorecards Tell Stories About Progress

Community Leadership Team members now have an effective way to communicate progress on each of the three priorities their regional team has identified.  County of San Diego staff provided technical assistance to help the Teams' design each Community Enrichment Plan and to monitor and track their progress with performance scorecards that are available on-line and shared at Community Leadership Team meetings. 

Each scorecard “tells the story” of the community change that is being sought (with population indicators) and the goals and objectives that represent the actions the Community Leadership Team has committed to (with performance measures). Performance measures reflect achievement compared to a target goal or convey the degree of progress on an activity compared to a schedule. A simple color-coding scheme is used to capture performance status relative to the target.

Homelessness Scorecard for North County Regions

Telling the Story: The CLT viewed homelessness as a growing crisis with serious implications, and the lack of affordable housing as a contributing factor. The narrative provides additional information as to why homelessness is a priority for the CLT, the strategy for change, and partners in getting it done.
What is Our Concern?: The rising numbers of persons experiencing homelessness, and residents spending more of their income on housing, are among the key population indicators that the CLT want to see change.

What Are We Doing About It?: Goals include expanding prevention and early intervention efforts among high-risk groups and strengthening education and advocacy for rent control. Objectives represent specific actions, such as developing a listing of resources to support high-risk populations and convening workshops to educate and engage community members about successful housing models.

How Are We Doing?: This Scorecard shows that North County Regions are behind schedule on some of their initiatives, such as compiling a list of shelters and beds in North County. The CLT is forming a work group to coordinate follow through on CEP goals and objectives. In this way, the Scorecard has helped reinforce need for more accountability for action.

Community Members Benefit

Through the five Community Leadership Teams, the County of San Diego launched a cycle of community planning in 2021 that has successfully led to the creation of five Community Enrichment Plans in 2024. Five Community Leadership Teams, each comprised of leaders from partner organizations and the community who represent the “voice of the community.” These plans and scorecards reflect the priorities for change but at the same time include objectives that are realistic, leveraging the capacities of partners through collective efforts.
