North County Community Leadership Team

Working Together to Improve North County Communities

Live Well San Diego Community Leadership Teams tie together the collective efforts of community groups in each region and provide a central point for planning and organizing collaborative action. The North County Community Leadership Team was formed in January 2012 to help guide planning for health, safety and thriving priorities in the region and to foster information sharing and connectivity among group members.

This collaboration includes community leaders, stakeholders and residents that are engaged in community enrichment efforts to help educate and mobilize communities, develop and address priority needs, identify resources and plan actions to improve the regions’ health and well-being.

These activities are informed by the North County Community Leadership Team's three priority areas: Mental Health, Substance Use, and Homelessness.

About North County Region

Map of North Coastal & North Inland Regions North County is the northern area of San Diego County, and consists of two regions with cities, communities, organizations and residents that work together to promote a healthy, safe and thriving region.

The North Coastal Region has a population of over 531,000 and consists of six cities (Carlsbad, Del Mar, Encinitas, Oceanside, Solana Beach and Vista) and over a dozen more communities that stretch geographically from Del Mar in the south to the Orange County border in the north and east to include Vista and Rancho Santa Fe. The US Marine Corps' largest installation, Camp Pendleton, is located in the North Coastal Region.

The North Inland Region has a population of nearly 597,000 residents living in four cities (Escondido, Poway, San Marcos and the northern part of the City of San Diego) and dozens of smaller communities. The region's diversity can be attributed to this vast geographic expanse, which includes suburban areas, remote desert communities, historic mountain towns, rural homes and farms, and Indian reservations.

Community Enrichment Plan

CHIP and Regional CEP with 2021-2022 Updates The Community Enrichment Plan (CEP) outlines an action plan to address several strategic focus areas that are based on community health, safety and thriving needs in North County identified by the Community Health Assessment. The CEP helps health officials and community members’ work together to set priorities, direct the use of resources, and develop and implement projects and programs to improve community health and wellness over the long term.

The CEP is continually guided by the North County Community Leadership Team, a group of community leaders who help to support the plan.

3-4-50: Chronic Disease in North County

3-4-50 are the three numbers that led to the inception of the Live Well San Diego vision and serve as the cornerstone of the Building Better Health strategy. Three behaviors contribute to four diseases that cause over 50 percent of deaths worldwide-- 53% and 56% of deaths in San Diego County’s North Coastal Region and North Inland Region respectively.

Community leaders have partnered with County representatives in the North County Regions to better understand these trends and to plan for ways to change health behaviors, which in turn could help to prevent these chronic conditions.

Community Health Assessment

CHIP and Regional CEP with 2021-2022 Updates A Community Health Assessment is a collaborative process of collecting information to help educate and mobilize communities, develop priorities, secure resources and plan actions to improve a region’s health. The Community Health Assessment in North County set the framework for developing the North County Community Enrichment Plan.

Demographic and Health Data by Region

The location of where people are born, grow, live, work, and age greatly influences their health and well-being. For example, built and natural environments may determine the food we eat, the quality of the air we breathe, and the healthcare services that are available to us, all of which are connected to health outcomes.

Exploring health data by geography is important to identify and address the needs of diverse local communities. Most of the demographic and health data for San Diego County is provided at the Health and Human Services Agency (HHSA) regional and subregional area (SRA) levels.

The most meaningful change is homegrown. Putting the North County Community Enrichment Plan into action requires local leadership as well as community engagement.
